To register for school at Zama Middle School you will need to complete forms for the Registrar and the Nurse. Follow the steps below to make the enrollment process go smoothly.
1. In order to prevent delay you should hand carry the following forms with you during your PCS. Up-to-date transcripts or school records Shot records Copy of your orders	2. You will need to call the school to set up an appointment with the Middle School Guidance Counselor. DSN 263-8914 Conus 011-81-46-407-8914
3. Print and Complete the needed forms below. Arrive 15-20 minutes before your scheduled appointment and drop off the registration forms to the registrar.	4. After your meeting with the guidance counselor stop by the Nurse's Office and turn in your complete medical forms.
Paperwork Needed For School Registration In order for your child to be OFFICIALLY ENROLLED in school, we MUST HAVE ALL off the following documents. The registrar or school nurse MUST be able to initial each blank that applies to your son / daughter’s enrollment. Printable copy of the registration check list from below.
*Forms for the School Registrar* One or more of the following: Current Orders and/for Extension Orders (if active duty) Family Entry Approval (if Navy) RAT Orders (if Civilians or Teachers) DSPA 2030.2 Student Eligibility DoDEA 600 Child’s Social Security Number (block “1f” of DoDEA 600 Form) DODEA MEMORANDUM: Eligibility for a DoDDS Education Request for Student Records Computer & Internet Agreement Computer Usage and Gaggle Email Agreement Safety & Security Information Permission for Publication & Field Trips and Publicity Permission Questionnaire for Race/Ethnicity and Home Language ESL Home Language Questionnaire Special Needs School Bus Registration *Forms for the School Nurse* Student Health History DoDDS Certificate of Immunization or Immunization Record (Please view to see that your child meets the requirements. Printing this form is not required.) Additional Forms which may pertain to your student Zama PTO Membership (Parent / Teacher Organization) Space Available letter (only for category 2, 3 and 4 students, usually non-military) Student Retain Control of Medications Hold Harmless Asthma History and Action Plan If your son or daughter is planning on playing school sports, you will need the following: Sports Physical (paperwork can be found at MEDDAC or with the nurse) Medical Power of Attorney (paperwork can be found in Main Office or JAG)
